Giving Back in Peralta

As the article in Health eNews mentions, Dr. Dutton joined other medical professionals with the non-profit organization, Community Empowerment Global Health Partnerships, for a medical mission trip to the mountainous village of Peralta in the Dominican Republic. While in Peralta, Dr. Dutton repaired birth defects and performed facial reconstructive surgery on local residents lacking care. The patients presented with a wide range of medical cases, and Dr. Dutton operated on 30 patients total, with each procedure lasting several hours.

The article points out that the village does have a general surgeon, but the majority of major medical care occurs with the help of Dr. Dutton and other physicians who volunteer to help out in Peralta, which has limited access to surgical care.

The article also highlighted the fact that this recent trip marks nine years that Dr. Dutton has been traveling to Peralta for medical missions. In fact, Dr. Dutton was reunited with a former patient whom he operated on more than eight years ago. At the time, the patient was just a child, and Dr. Dutton had performed a cleft palate repair. After hearing about Dr. Dutton’s return to Peralta, the article states that the patient drove two hours to personally thank Dr. Dutton for the surgery he performed years ago.

Now, patients who want to get rid of their double chin without going under the knife, have a new, non-surgical option.

The Food and Drug Administration has approved the use of Kybella^® (deoxycholic acid) which is injected into the fat beneath the chin to decrease the bulk and tighten the skin. Kybella^® contains a natural molecule that is already present in our bodies that dissolves fat and even prevents it from further accumulating.

“This treatment has worked wonders for patients who have moderate to severe fat build-up, or submental fat, under their chin,” explains Dr. Jay Dutton of The Rhinoplasty Institute of Chicago in Oak Brook and Tinley Park, Illinois. “One of the most exciting things about Kybella^® is that after two to four treatments, the results will last even if the patient gains weight in other areas.”

Little Downtime

Most sinus infections (acute sinusitis) can be treated with antibiotics or relieved with home remedies, such as vaporizers, decongestants or saline nasal drops. However, in more serious cases (chronic sinusitis), for which antibiotics are not effective in opening up the sinuses, or if there is a structural abnormality, undergoing surgery can provide relief and even end sinusitis for good.

“Having a chronic sinus infection is one of the most frustrating things because conservative treatment, such as oral antibiotics, doesn’t seem to help,” explains Dr. Jay Dutton of The Rhinoplasty Institute of Chicago in Oak Brook and Tinley Park, IL. “Sinus surgery can help, but symptoms can recur after a year or two and 25 percent of patients will need a second surgery to reopen their scarred over sinuses.”

Non-surgical rhinoplasty is a procedure in which injectable fillers are used to alter the appearance of the nose. These fillers can fill in depressed areas, smooth the appearance of a bump and even help lift or adjust the tip of the nose.

“Using fillers to adjust the shape of the nose is a tool that can be utilized for very limited and isolated changes,” explains Dr. Jay Dutton of The Rhinoplasty Institute of Chicago in Oak Brook and Tinley Park, IL.

By injecting fillers into specific areas of the nose, volume is added and the structure of the nose can be molded according to the patient’s desires. Drooping tips may appear to be lifted, nostrils can be made to appear smaller and crooked noses may appear straighter.

The specific type of filler used will determine how long the results last. Typically, they last up to 14 months. Because this in a non-invasive procedure, no general anesthesia.  However, there can be complications, such as skin necrosis (“death”) from a poor blood supply to the skin.

“It is important to speak to a trusted and experienced doctor before exploring the option of fillers because there can be some side effects – like any procedure. “Occasionally, a young patient will come into my office with a parent to discuss nasal surgery to create a new look for their nose,” explained Dr. Jay Dutton of the Rhinoplasty Institute of Chicago (Oak Brook and Tinley Park, IL). “I don’t typically recommend patients under 16 undergoing rhinoplasty for various reasons, but there are exceptions.”

Recently, a 16-year-old patient named Victoria was one such example.

“I broke my nose when I was 11 which caused a large bump on the bridge,” Victoria explains. “I also had chronic sinus infections for years, so when I went to Dr. Dutton for sinus surgery, I opted to have the bump on my nose fixed as well.”

Most often, young patients undergo rhinoplasty surgery because of an accident or a sports injury. “Because of the bump, Victoria was self-conscious about the bump on her nose,” explains Dr. Dutton. “I knew that rhinoplasty would improve the look of her nose and even out the features on her face.”

However, when a young patient simply doesn’t like the way his or her nose looks, an immediate rhinoplasty may not always be the best solution. Young people are still maturing physically, not only the facial bones, but also the frontal lobe of the brain, which can make them more impulsive than mature adults.

The physical appearance of a young person’s face changes over time. Around age twelve, a person’s maxillary sinuses start to develop, which elongates the face and can also affect the shape of the forehead. For most girls, the nose has finished growing by age 15 or 16 and for most boys, it has finished growing by 16 or 17.

“Over time, some young patients actually grow into their’ nose or decide that the bump in their nose is a unique feature that they embrace,” explains Dr. Dutton. “I understand how difficult it can be to want to change something that makes you feel uncomfortable, but sometimes waiting a few years can make a patient more content in the long run.”

One thing is certain: a patient of any age should consult with a qualified, board certified plastic surgeon that has performed many rhinoplasty procedures. Patients should always review their photos with their surgeon in advance to make sure their goals are aligned.  They may also benefit from a detailed analysis with 3-D computer software.

“Dr. Dutton is not only a highly skilled surgeon, but also is a very kind and helpful person,” explains Victoria. “He always made me feel comfortable and answered all my questions. He really showed that he genuinely cared about me.”
